Create a form

1

Start

Click New form and pick a starting point: Blank, Booking (name, email, then a meeting time), Lead (name, email, phone, budget), Consent (a text and a checkbox), or From an agent (a copy of a qualifier or appointment agent’s questions).
2

Settings

Give it a Name and choose the WhatsApp account it belongs to and the Form language (the language of WhatsApp’s own texts in the form). Set the Button in the message that opens the form (up to 30 characters), and the Next button and Send button labels.
3

Screens and blocks

A form has up to 10 screens. Add blocks to each one:
  • Content: Heading, Text, Image (a JPEG or PNG up to 10 MB, resized for WhatsApp; up to 3 per screen and 15 per form), and Consent (a checkbox with an optional “Read more” link to your terms).
  • Questions: Short answer, Long answer, Number, Email, Phone, Date, Single choice (up to 200 options), Multiple choice (up to 20), Yes / no, Photo upload, and Document upload (how they work).
  • Dynamic: Meeting slot, a day and time picker that books on your calendar. It needs a booking calendar, and a form holds one.
Text can be bold, italic, struck through, or a bulleted or numbered list. A Heading, a Text, and a question’s Question text can include contact details: type {{ or use the variable button, as in templates. Each customer sees their own values. A detail the contact doesn’t have is left out, so write the sentence so it still reads well without it.
4

Set up each question

Write a short Label (up to 20 characters, 30 for choices, 40 for dates) and, if you need more words, the Question text shown above it. Choose Save the answer to a contact property: the question then takes the property’s type and options. Each property can be used once per form. A file property can only take the answer of a photo or document question. Mark it Required if needed, and leave Prefill with what the contact already has on so customers only confirm what you know.
5

Publish

Save draft keeps your work without sending it to Meta. Publish sends it to Meta, which checks it in seconds (there is no human review). Once it is Published, Preview in WhatsApp opens Meta’s preview of the form.
The phone preview on the right shows each screen as you build it.

Statuses

A red dot means Meta reported a problem with this form. Used by lists the agents and templates that use the form. Not used means nothing sends it yet: add it to an agent (how) or a template. Forms marked From agent belong to a qualifier or appointment agent: they open read-only, and you change them on the agent. A form marked From surveys is your satisfaction survey: it is read-only too, and you change it under Settings, then Surveys.

Edit a published form

At Meta, a published form can’t change. When you edit one in Ciarem, it goes back to Draft until you publish again, and publishing creates a new version. Messages already sent keep opening the version they were sent with. While your edit is a draft, agents and the Inbox don’t send the form; templates keep sending the last published version. So publish soon after you edit. You can’t delete a form while an agent or a template uses it: remove it from them first.

Add a form to a template

In the template editor, add the button Open a form and choose a published form of the same WhatsApp account. A template carries one form, and authentication templates can’t carry one. Each recipient gets their own copy, also in broadcasts, and this is how to send a form when the 24-hour window is closed. When you publish a new version of a form, the templates using it that are approved, rejected, or paused are sent back to Meta for review, and can’t be sent until approved again. Ciarem asks you to confirm first. Templates already in review keep the previous version.
